Patrick Henry Enrollment and Registration Information
 
District and Registration Forms
  • All 6th, 7th and 8th grade students must complete annual district forms and bring them to our August Orientation or deliver to the Attendance Office by the first day of school.
  • New students must complete Registration Forms as part of the enrollment process along with annual district forms.

 

Resident School Enrollment
  • Students whose home address is within our school boundary automatically qualify to enroll at Patrick Henry.

  • Non-Resident School students must complete enrollment forms.

  • Please contact our Attendance Office for more details at (818) 832-3889.

Step 3
Create an LAUSD Parent Portal account
Crear una cuenta el Portal de Padres de LAUSD 
Step 4
Upload your complete Student Enrollment packet documents into the LAUSD Parent Portal. If possible, please upload documents separately for each enrolling student.
 
Once you have uploaded and submitted documents in the LAUSD Parent Portal, the LAUSD Enrollment team will be notified and will contact you to review documents and support next steps.
 
Alternatively, if you do not have access to the internet, beginning Monday, April 20, 2020, you may call Student Health and Human Services (213) 241-3840 (press 6 for student enrollment) to request support.
Suba el paquete completo de Inscripcion del Estudiante y los documents en el Portal de Padres de LAUSD (haga clic aqui para instruciones).  Si es possible, favor de subir los documents separados para cada estudiante que se esta inscribiendo.
 
Una vez que los documentos has sido sometidos en el Portal de Padres de LAUSD, el equipo de Inscripcion de LAUSD sera notifcado y se comunicaran con usted  para revisar los documentos y proveerles apoyo con los siguientes pasos.
 
Alternativamente, si no tiene acceso a Internet, a partir del lunes 20 de abril de 2020,favor de llamar a la oficina de Salud y Servicios Humanos del Estudiante al (213) 241-3840 (presione el 6 para inscripcion del estudiante) para pedir asistencia.
Step 5
Expect a call from a school-site and/or other LAUSD employee, who will assist with scheduling an appointment to complete your child’s enrollment process.  During the appointment you will be asked to provide photo identification, sign enrollment related forms, and may be giving additional forms and materials to support virtual/remote instruction.

Espere una llamada de un empleado de la escuela u otro empleado del LAUSD, que lo ayudará a programar una cita para completar el proceso de inscripción de su hijo. Durante esta cita, se le pedirá que proporcione una identificación con foto, firme formularios relacionados con la inscripción y es posible que se le proporcionen formularios y materiales adicionales para apoyar la instrucción virtual / remota.
